Most of the time, your doctor or hospital handles the claims process automatically — they bill UnitedHealthcare directly, and you simply receive an Explanation of Benefits (EOB) in the mail afterward. But there are situations where you’ll need to file a claim yourself, and knowing exactly how to do it can mean the difference between a smooth reimbursement and a frustrating delay. This guide walks you through every step.

When Do You Need to File Your Own Claim?

Common situations where you file your own claim

  • Out-of-network care: You saw a provider who doesn’t participate in your UHC network and paid out of pocket.
  • International or emergency travel care: You received medical services outside the United States.
  • Your provider didn’t file on your behalf: Some smaller practices or alternative-medicine providers may not submit claims to insurers directly.
  • Reimbursement for medical supplies or equipment: You purchased an eligible item without going through an in-network supplier.
Don’t miss your filing deadline. UnitedHealthcare requires that claims be submitted within a specific timeframe after the date of service — typically 90 days to 1 year, depending on your plan. Check your plan documents for the exact deadline. Claims submitted after this window may be denied entirely, regardless of whether the service was covered.

What You’ll Need Before You Start

Gathering the right information before you begin will make the process much faster. Have the following ready:
  • Your Member ID (from your UHC ID card)
  • Provider details: full name, address, phone number, and NPI (National Provider Identifier)
  • Date(s) of service
  • Diagnosis code(s) (ICD-10): your provider can supply these
  • Procedure code(s) (CPT): also available from your provider or the itemized bill
  • Itemized bill or receipt: a detailed statement showing each service and its charge (not just a summary receipt)
  • Proof of payment if you’re seeking reimbursement for an amount you’ve already paid

Filing a Claim Online

The fastest way to submit a claim is through your UHC member account at uhc.com.
1

Sign in to your member account

Go to uhc.com and log in with your HealthSafe ID. If you haven’t registered yet, visit activate.uhc.com to create your account first.
2

Navigate to Claims

From your member dashboard, select Claims & Accounts, then choose Submit a Claim from the dropdown menu.
3

Select the type of claim

Indicate whether this is a medical, dental, vision, or prescription drug claim, and confirm whether you are filing for yourself or a covered dependent.
4

Enter provider and service details

Fill in the provider’s name, address, NPI, date(s) of service, diagnosis codes, and procedure codes as listed on your itemized bill.
5

Upload supporting documents

Attach a clear, legible copy of your itemized bill and any proof of payment. Accepted formats are typically PDF, JPG, or PNG.
6

Review and submit

Double-check all entries for accuracy, then click Submit. You’ll receive a confirmation number — save it for your records.
7

Track your claim

Return to the Claims & Accounts section at any time to check the status of your submission. Processing typically takes 30 days once all required information is received.
You can also file a claim through the UnitedHealthcare mobile app. The process mirrors the online portal, and you can use your phone’s camera to photograph and upload documents on the spot.

Filing a Claim by Mail

If you prefer to submit a paper claim, you can download the UHC Medical Claim Form directly from your member account under Forms & Documents, or request one by calling the Member Services number on the back of your ID card. Mail your completed form and supporting documents to:
UnitedHealthcare Claims P.O. Box 30555 Salt Lake City, UT 84130-0555
Always send paper claims via certified mail or a trackable carrier so you have proof of submission. Keep photocopies of everything you send.

Understanding Claim Processing Timelines

Once a claim is processed, you’ll receive an Explanation of Benefits (EOB) summarizing what UHC paid, what was adjusted, and what — if anything — you owe the provider.

What to Do If a Claim Is Denied

A denial isn’t the end of the road. Review your EOB carefully for the denial reason code, then:
  1. Contact your provider to confirm the correct codes were used.
  2. Call the Member Services number on your ID card to request clarification.
  3. Submit a formal appeal through your member account if you believe the denial was in error — you generally have 180 days from the denial date to appeal.
Filing your own claim takes a little preparation, but the process is straightforward once you have the right information in hand. When in doubt, the UHC Member Services team is always available to guide you through it.
