Understanding how your health insurance claims work can save you time, money, and frustration. Whether you're checking on a recent claim, dealing with an unexpected bill, or need help getting reimbursed for an out-of-pocket expense, the answers below cover the most common claims-related questions from UHC members. For claim-specific inquiries not addressed here, contact Member Services using the number on the back of your card.

<AccordionGroup>
  <Accordion title="How long does it take for a claim to be processed?">
    Most claims submitted by in-network providers are processed within **14 to 30 days** of receipt. Claims that require additional review — such as those involving prior authorization, coordination of benefits with another insurer, or complex medical coding — may take longer. Once a claim is processed, you'll receive an **Explanation of Benefits (EOB)** by mail or electronically (if you've opted into paperless statements) detailing how the claim was applied to your benefits.
  </Accordion>

  <Accordion title="How do I check the status of a claim?">
    Sign in to your account at [activate.uhc.com](https://activate.uhc.com) and navigate to **Claims & Accounts** or **Claims History**. You'll see a list of recent claims with their current status: received, in process, paid, or denied. You can also view full claim details and your EOB from this screen. Alternatively, open the UHC mobile app and tap **Claims** from the bottom navigation bar for a quick status overview. If a claim is not appearing, see the FAQ below about provider billing delays.
  </Accordion>

  <Accordion title="What does it mean when a claim is denied?">
    A denied claim means UHC did not pay — or did not fully pay — the amount billed by your provider. Common reasons for denial include: the service is not covered under your plan, a required **prior authorization** was not obtained, the provider is out-of-network and your plan has limited out-of-network benefits, or the claim was submitted with incorrect coding. When a claim is denied, you'll receive an EOB explaining the specific reason. You are not necessarily responsible for the full billed amount — review your EOB carefully and consider filing an appeal.
  </Accordion>

  <Accordion title="How do I appeal a denied claim?">
    You have the right to appeal a denied claim. To start, review the denial reason on your EOB. Then, sign in to your account and go to **Claims → File an Appeal**, or send a written appeal to the address listed on your EOB. Include a copy of the EOB, a letter explaining why you believe the claim should be covered, and any supporting documentation (such as a letter of medical necessity from your provider). UHC is required to respond to standard appeals within **30 days** and urgent appeals within **72 hours**. If your internal appeal is unsuccessful, you may have the right to an external review by an independent organization.
  </Accordion>

  <Accordion title="Why is my claim showing a patient responsibility amount I didn't expect?">
    Your patient responsibility reflects what you owe after your plan benefits have been applied. Unexpected amounts can arise from several factors: your annual **deductible** may not yet be met, the service may be subject to a **copay or coinsurance** under your plan, the provider may have billed for services beyond the covered scope, or a coordination of benefits calculation with a secondary insurer may have adjusted the totals. Review your plan's Summary of Benefits and Coverage (SBC) document — available in your member portal — to understand your cost-sharing obligations for the services you received.
  </Accordion>

  <Accordion title="What is the deadline for submitting a claim?">
    In most cases, claims must be submitted within **12 months** of the date of service, though your specific plan may have a different timely filing limit. Check your plan documents or Summary of Benefits and Coverage for your plan's exact deadline. If you're submitting a claim yourself (for example, for reimbursement of an out-of-network expense), download the **UHC Medical Claim Form** from the member portal, complete it, and submit it with your itemized receipt before the deadline.
  </Accordion>

  <Accordion title="How do I get reimbursed for an out-of-network expense?">
    If you paid out of pocket for a covered service from an out-of-network provider, you can submit a claim for reimbursement. Download the **UHC Member Reimbursement Form** from your member portal under **Claims → Submit a Claim**. Attach an itemized bill from your provider showing the date of service, type of service, provider's name and NPI number, and the amount charged. Mail or upload the completed form and documentation as instructed. Reimbursement will be issued at the out-of-network benefit rate defined in your plan, minus any applicable deductibles or coinsurance.
  </Accordion>

  <Accordion title="My provider says they billed UHC — why isn't the claim showing up?">
    There are a few reasons a claim might not appear in your account yet. First, allow **7 to 10 business days** after the date of service for the claim to be received and entered into the system. Second, verify with your provider that they billed using the correct Member ID and group number from your current card. If your coverage changed recently, an old member ID may have been used. If the claim still doesn't appear after two weeks, contact Member Services with the provider's name, date of service, and any reference number the provider gave you.
  </Accordion>

  <Accordion title="What is prior authorization and do I need it?">
    **Prior authorization (PA)** is advance approval from UHC confirming that a specific service, procedure, or medication is medically necessary and covered under your plan before you receive it. Not all services require prior authorization, but many specialist visits, surgeries, imaging studies (such as MRIs), and specialty medications do. Your provider typically handles the PA request, but it is your responsibility to confirm that authorization has been obtained before receiving care. You can check whether a service requires prior authorization by signing in to your member portal and using the **Coverage & Benefits** tool, or by calling Member Services.
  </Accordion>

  <Accordion title="How do I get an itemized bill from my provider?">
    Contact your provider's billing department directly and request an **itemized statement** or **itemized bill**. This document lists each service, supply, or medication billed during your visit with the corresponding medical billing codes (CPT codes) and individual charges. An itemized bill is different from a summary bill or receipt. You may need it to submit a reimbursement claim, dispute a charge, or verify that you were billed correctly. Providers are generally required to provide this document upon request, often within a few business days.
